Rolling waves of consolidation have significantly decreased the number of hospital systems in the U.S., leading to dominant regional systems. Increased concentration potentially affects industry quality, prices, efficiency, wages, and more. Much of the consolidation research is focused on merger events and estimating effects on the merged entities. In contrast, our new working paper is not based simply on merger data but takes account of the overall increase in consolidation across the country without respect to cause.

Specifically, we use the intensity of changes in hospital system consolidation in metropolitan statistical areas (MSAs) over two periods to estimate its effect on the wage growth of higher-earning professional workers—in this case registered nurses. We focus on registered nurses as a homogeneous group of workers with some degree of industry-specific education and skills. Registered nurses represent the largest single occupational classification in hospitals and urgent care centers, representing one in four workers.

Understanding the dynamics of local healthcare labor markets is critical given the importance of the sector for the U.S. economy; even more so in the wake of the pandemic amid continued uncertainty around long-term effects (e.g., early retirements, career shifts, education delays). Moreover, labor shortages among hospital-based nurses, which may be a symptom of monopsony, have been endemic in the industry for many years. The wages of nurses were stagnant between 1995 and 2015 despite increasing demand for healthcare over the same timeframe even as it was the only sector that added employment during the Great Recession. Explanations for the stagnation of nurse wages—in one of the more highly unionized professional occupations in the country—are not readily apparent.

In The Fight Against COVID, Health Workers Aren’t Immune To Vaccine Misinformation

As new data shows 1 in 500 Americans has died from COVID-19 and the delta variant continues to surge across the country, the next challenge many health care leaders face is within their own staffs: the 27% of of U.S. health care workers who have not been vaccinated against the disease as of July, according to a study by The COVID States Project.

On top of that, other research shows that since the vaccine first became available to health care workers in December 2020, the rate of vaccination among nurses and nursing home aides has been lower than that of physicians. This may be of particular concern because nurses and aides have such frequent and close contact with patients.

Data shows health care workers have gotten the COVID-19 vaccine at a higher rate than the general population: 73% versus 64% of non-health care workers. And many may assume that people who work in health care industry are more enthusiastic about the vaccine and less apprehensive.

While a majority of nurses are vaccinated and more than half support vaccine mandates in the workplace, some are pushing back against requirements to get vaccinated or face mandatory testing and say they would rather leave their jobs. And hospitals are already feeling the effects.

55% of Ontario’s nursing home workers opted for vaccine

Despite the fact long-term care workers were the first in Ontario invited to get the COVID-19 vaccine last December, a little more than half of them have volunteered to get the shot.

As of this week, only 55,000 of 100,000 long-term care workers in Ontario have been inoculated, according to the province’s Ministry of Health.

Dr. Hugh Boyd, chair of the Ontario Medical Association’s section on long-term care and care of the elderly, said a lack of confidence in the vaccine and pervasive myths about the quick development and safety of the shot is at the root of the low numbers.
